Convenient Appointment Scheduling
Our courteous staff is available to schedule your exam. We offer two convenient options to schedule your non-urgent appointment:
- Online: Complete our secure online appointment request form. Someone from Borg Imaging will contact you by phone to confirm your appointment request.
- By Phone: Call (585) 271-0401, Monday – Friday, 8:00 am – 5:00 pm.
Please Note: If your health insurance requires a prior authorization for your exam, please ensure this is in place prior to your appointment with us.
Please bring the following with you:
- Your ordering physician’s form/requisition.
- Name and address of any other physician(s) you would like to receive a copy of your report.
- Any health updates, medications, etc.
- Your insurance card.
- Your co-payment, if your health insurance requires one.
